Transition marks a developmental stage to independence. Youth leave special education services and plan for employment or education leading to employment. The transition process can seem overwhelming given the wide array of services available to adults with disabilities.

This site presents information to prepare for transition. Each section includes checklists, national initiatives, as well as information for obtaining services in your area. This cite is not intended to be comprehensive. However, it is hoped that the information presented will assist youth, families, and professionals to engage in the process of transition collaboratively.
